Of Derby, NY, passed away Saturday November 22, 2025 in Eden Rehab and Nursing facility, at the age 77. Beloved husband of Sandra Falkowski. Loving father of Christopher Falkowski, Perry Heck, Lisa (Kyle) Boyce and Gina (Mark) Behringer. Brother of Edward (Christine) Falkowski. Also survived by 8 Grand and 3 great grandchildren. A celebration of David’s life will be held at a future date to be announced. Arrangements by the ADDISON FUNERAL HOME, INC. Angola, NY
